ACNE – program of treatment in accordance with the newest dermatological consensus

There are a number of methods of acne treatment based on various strategies created by international expert groups, however, each acne is a bit different and each patient should be consulted individually. Each method of treatment, even if it brings short improvement, must be applied based on many-months’ or even many-years’ supportive procedures. Depending on the form and intensification of acne we can apply topical, general and physical treatment, and we frequently recommend concomitant therapy.

ACNE – TREATMENT PROGRAM

1. Medical consultation:
 - finding the essence of the disease, that is making the patient aware of the complex etiopathogenesis and 
   of the necessity of lengthy or supportive treatment.
2. Discussion of care at home:
- individual selection of suitable dermocosmetics depending on the skin type,
- washing off make-up, cleaning the face,
- peelings.
 3. Treatment:
 Depending on the acne type:
 - external therapy – usually two or three drugs (modern retinoids, benzoyl peroxide, antibiotics),
 - oral therapy (antibiotics, isotretinoin orally, hormones),
 - topical treatment (should cover all etiopathogenetic causes so a few drugs are used simultaneously):

Retinoids
They hinder formation of comedones and secretion of sebum, thus limiting inflammation. They can cause skin irritation, therefore, in case of sensitive skin they should be combined with creams.

Benzoili peroxydum
It influences bacteria and prevents resistance to antibiotics; it has a peeling effect that deeply clears the pores. It protects skin from staphylococcus bacteria and yeasts. Unfortunately, it frequently leads to irritation.

Antibiotics
Antibiotics are applied only in case of inflammation, never in monotherapy, always together with retinoids, benzoili peroxydum or azelaic acid. Antibiotics cannot be used lengthily in order to prevent relapses.

Azelaic acid
It has an anti-bacterial effect, hinders melanogenesis while fighting off post-inflammatory discolorations.

GENERAL TREATMENT

1. Antibiotics
Antibiotics are applied in intensive acne or intolerance to topical drugs. They cannot be taken for more than six months, however, too short application of the drug is not advisable either – the minimum is sixteen weeks / doxycycline, limecycline, tetracycline.

2. Isotretinoin orally
It has been used in treatment of acne since 1979, therefore, we know its effects on the organism well. Isotretinoin affects all etiopathogenetic factors: it significantly diminishes the size of sebaceous glands and production of sebum, normalizes keratinization of hair follicles, prevents development of comedones and microcomedones as well as exerts direct anti-inflammatory activity and hinders development of bacteria. At present isotretinoin applied orally is the most effective method of acne treatment with the registered index of remission in over 80% of patients.

3. Hormonal drugs
This method of treatment is only for women that have hormonal changes documented by appropriate examination. In hormonal treatment contraceptive pills are used, too. They do not cure acne, but during their application its symptoms diminish. The results of application of hormonal drugs can be seen after two–three months; unfortunately, not everyone is receptive to this method of treatment.

4. Supportive therapy
After completed treatment of acne it is always advisable to start supportive therapy.

5. Medical procedures
Supportive therapy accelerating treatment of acne:
a) MEDIDERMA chemical peelings: Melaspeel K,  Melaspeel KH,Melaspeel J, Salipeel, Ferulac, Retises CT, Azelac;
b) microdermabrasion is advised in combination with acids and mechanical cleaning of the skin.
6. Treatment of post-acne scars,  fighting complications following completed pharmacological therapy:
a) chemical peelings intensely combined with microdermabrasion,
b) USA production PEARL FRAXEL LASER – it is a semi-ablative laser of the newest generation based on the YSGG technology, designed for treatment of:
- scars (e.g. hypertrophic post-acne scars – keloids, burns),
- wrinkles,
- symptoms of skin photoaging (discolorations),
- stretchmarks in combination with mesotherapy,
- prevention of skin aging and preservation of its young appearance.

Suitably selected length of the waves and use of a laser scanner allows for surface smoothing of the damaged skin and triggering the process of creation of new collagen. It makes the skin shrink, gain younger, fresher appearance; its texture improves, small wrinkles and scars disappear. Skin revival is visible after the first procedure. While it is heated, the epidermis is protected by use of the cooling sapphire tip that reduces discomfort by controlling the heat flow inside of the skin. As a result, the method is safe (the laser does not affect surrounding blood vessels or nerves). Reddening visible after the procedure is maintained for a few days, after about three days skin starts to peel off and after ten days the first effects of the procedure are be visible.

The effect after a procedure with use of Pearl Fraxel laser:
- smoothed wrinkles,
- smoothed skin unevenness,
- improved skin texture,
- reduced pores,
- unification of the skin color,
- removal of changes caused by the sun and photoaging,
- removal of post-acne scars,
- removal of stretchmarks.
