CELLULITE (PEFS – PANNICOLOPATIA EDEMATO-FIBRO-SCLEROTICA, LIPODYSTROPHY)
So far cellulite has been perceived solely as an esthetic defect. At present it is believed to be a chronic inflammation affecting the deep layer of the dermis, fat tissue under the skin, connective tissue, peripheral lymphatic and circulatory systems. Etiopathogenesis of this disorder is complex in nature and encompasses the following:
- skin lymphatic circulation disorders,
- dermis swelling caused by excessive synthesis of improper collagen and elastin fibers in the blood vessels,
- enlargement of fat lobules within the subcutaneous tissue as a result of excessive accumulation of fat cells,
- fibrosis in the fibromuscular septa of the subcutaneous tissue and the deeper layer of the dermis,
- increased accumulation of glycosaminoglycans – substances keeping water in the tissue.

Cellulite is, in other words, lipodystrophy, that is inappropriate location of fat tissue combined with tumid and fibrous changes of the subcutaneous tissue.
Cellulite, a common name for this skin disease characterized by creation of uneven swellings, lumps under the skin, sometimes painful ones resembling “orange peel”.
HOW IS “ORANGE PEEL” FORMED?
The mechanism of formation of lumps under the skin in case of cellulite is simple. Subcutaneous tissue is composed of fat cells, groups of which – lobules – are divided by fibrous septa. In women the septa are perpendicular to the skin, which causes enlargement of fat lobules visible in the upper skin layers in the form of lumps giving an impression of skin unevenness – the “orange peel” effect.
In all women the physiological structure of fibrous septa of the subcutaneous tissue predisposes them to have cellulite. In men the septa are crosswise and constitute a fixed scaffolding for fat lobules, preventing creation of lumps in the dermis.
Cellulite in women appears mostly in the areas of:
- hips,
- abdomen,
- outsides of thighs,
- insides of knees,
- buttocks.
Factors influencing cellulite formation:
Genetic:
- female gender,
- skin color (mostly white),
- location of fat tissue (accumulation of fat tissue mostly on the abdomen and hips),
- inclination for venous insufficiency.
Hormonal (mostly excess of female hormones – estrogens):
- estradiol causes accumulation of fat in the subcutaneous layer in the area of abdomen, buttocks, thighs,
- insulin and prolactin stimulate growth of fat cells and synthesis of lipids in the cells.
Environmental:
sedentary lifestyle:
- decrease in the muscular mass in favor of fat tissue,
- tendon and muscle flaccidity,
- decrease in efficiency of the muscular pump in the lower limbs (liquid stasis),
- wearing tight clothes (it makes blood flow difficult),
- wearing high-heeled shoes (it enhances calf muscle dysfunction, makes blood flow difficult),
bad eating habits:
- excessive salt intake (water accumulation),
- excessive fat and carbohydrate intake,
stimulants:
- cigarettes – creation of free radicals,
- alcohol increases fat creation,
emotional disorders:
- they cause an increase in the concentration of hormones intensifying fat creation in the organism.
CELLULITE STAGES:
1st phase:
After grabbing the skin as a fold small-degree furrows are visible. It is an effect of venous blood stasis and an increase in the permeability of venous vessels.
2nd phase:
There are hollows – an image of “orange peel”. In this phase swelling appears.
3rd phase:
Hollows are visible while flexing muscles; we speak of the so-called “orange peel” spontaneous symptom.
4th phase:
Appearance of the so-called “leopard skin” effect. We can observe numerous small lumps. This is intense pathology pertaining to all elements of cellulite: venous vessels, fat cells, connective tissue.
5th phase:
Numerous, large lumps. Complete pathology, decreasing life comfort due to pain in the altered places.
Cellulite prevention:
- physical movement,
- diet rich in vegetables and low in fats,
- drinking at least two liters of water a day,
- protection of the fat tissue from microtraumas (avoiding tight clothing),
- ensuring appropriate venous and lymphatic microcirculation through rational physical effort (aqua aerobic, swimming, walks),
- maintaining appropriate body weight.
